Summary

The Illinois Early Intervention Report Format provides an overview of the Illinois Early Intervention Report Format, including required elements of the report and related policies and procedures. Examples and strategies to support early interventionists in crafting high quality early intervention reports are also provided.

  • Recall where to access the latest report format.
  • Describe when the written report required for evaluation/assessment or assessment must be submitted.
  • Recall to which entity a written report of findings must be submitted.
  • Describe the qualities of a well written report.
